Identity fraud
I have an account with my daughter's name
on it as she uses the debit card to buy
things for me. When I opened the statement
a few months go, someone had used the card
to purchase over $800 worth of merchandise.
We don't know how they were able to do it
as she was very careful with the card. The
bank replaced the money withdrawn by the
user, thankfully.

Fraud
While I was in Iraq (as a navy hospital corpsman in a marine infantry
battalion, near Baghdad), someone withdrew $1,000 from my checking
account. My bank website said that someone made two ATM withdrawals,
for $500 each, but I thought that the only ATM card, for that account,
was with me. I called the bank, and I couldn't find a bank employee who
knew how that happened. They replaced the $1,000.

identity theft
No. But i did have a business refuse to take a rather large check
because the check guarantee service they used evaluated the risk based
on phone #. Turns out a down and out was using my phone number to do
the limited business he was able to initiate. I was getting calls from
banks because he was always overdrawn. I did not lose monetarily, but
it was still inconvenient.
