ASSISTED TRANSPORTATION - (1-one way trip) Provision of assistance, including escort to a person who has difficulties (physical or cognitive) using regular vehicle transportation.LEGAL ASSISTANCE - Provision of legal advice, counseling and representation by an attoney or other person acting under the supervision of an attorney.
MATERIAL AID - Issuing assistance devices and other goods, e.g., walkers, wheelchairs, fans, commodities, personal hygiene items.
OUTREACH - A personal contact to identify clients and to encourage use of existing services.
TELEPHONING - Contacting individuals by phone on a routine basis to determine physical status, to provide comfort and help.
TRANSPORTTION - Transport clients to doctors, pharmacies, grocery store, etc. Arrangements need to be made one day in advance.
VISITING - Visiting in a home of older individuals providing comfort, encouragement, listening, fellowship, etc.
HOMEMAKER - Preference given to persons living alone, not in a good health, homebound and needy. Services limited to one or two hours according to need assessment. (Funded by United Way).
CONGRETE MEALS - Hot meals served five days a week at two different sites.
