CASE MANAGEMENT
Case Management is conducted by the case manager who will begin with the needs assessment interview to determine what the client needs. The case manager may help prioritize the needs and clarify the difference between needs and wants. The case manager has knowledge of local resources such as: The American Red Cross, Salvation Army, The Alliance for Aging, Deaf Services, and so on, in order to refer clients for help.
If the client has needs for which a simple referral will not suffice, the case manager will bring a request to the FRIEND Board of Directors. It is the responsibility of FRIEND to identify resources to meet that unmet need.

All cases will require documentation and release of confidential information signed by the client which enables FRIEND to both release and obtain confidential information that is relevant and necessary for the purpose of providing assistance to the client.

The case manager will work to empower the client to be active in their own recovery. The case manager may help the client identify the choices available to them, but decisions about the recovery effort are made by the client.
