REPORT OPTION: Fill in, then print, OR print out, then write in. Send regular mail along with Receipts/Expenses report,
any receipts, and a matching funds check IF it was a fund raiser.

Complete the Report and Receipt/Expense Report found below,
attach any receipts, and if yours is a Fund Raising event,
include a check to receive your matching funds.
