Those who receive assistance must complete a Permission to Disclose Information form. This signed form provides Thrivent Financial permission to use, disclose and publish the recipient’s name and other related information about the recipient for the purpose of planning, conducting, advertising and communicating the results of an activity for the recipient’s benefit. | |

A parent, guardian or other legal representative of a minor child or incompetent recipient (one who receives the help) must complete a "Permission to Disclose Information" form. This completed form provides Thrivent Financial for Lutherans with permission to use, disclose and publish the recipient's name and other pertinent information about the recipient for the purposes of planning, conducting, advertising and communicating the results of a fund-raising activity for the recipient's benefit (e.g., to publicize the fund-raising activity). | |
