Report for Funding Activity Results (.doc format)
The Report for Funding Activity Results is to be submitted to the chapter Financial Director upon completion of any Care Program activity. If you have conducted a Care In Congregations® activity or a Care Abounds In Communities® activity, you need to submit this form.

Complete this all-purpose form as appropriate for your activity and mail it along with receipts for expenses, if any, and check(s), if a fundraiser, to the chapter Financial Director:

Care in Congregations Activity Tracking Form (Optional)
The Care in Congregations Activity Tracking Form is intended to help
congregational coordinators keep track of the information they need
in order to request and report activities. It should NOT be submitted
to the home office as a funding request.

Congregational coordinators should request funding and report results
either on-line, by calling Fraternal Customer Service (1-800-236-3736) or by calling Judy Marinko (1-440-427-1430 or 1-866-734-0330).

Acknowledgement of Donation Received form
The Pension Protection Act of 2006 includes tax-deduction legislation that impacts our chapter volunteers.

Beginning in 2007, all monetary donations require some sort of verification to be tax deductible. This means more donors will need and will request written acknowledgements
or receipts for donations made at chapter activities. Service teams must provide donors with a completed Acknowledgement of Donation Received form for all donations of $25 or more at a chapter activity. For donations of less than $25 ask donors if they'd like an Acknowledgement of Donation Received form.
