Care Abounds in Communities Request for Funds Form |
Care Abounds in Communities supports fund-raisers and hands-on service projects to benefit individuals, families and non-profit organizations (both Lutheran and non-Lutheran).To obtain a copy of the "Funding Activity Request" form, click the icon to the right. When you are done viewing/printing the form, click on your "back" button to return to the main site. |

Receipts, Funds Raised & Expenses Report |
Receipts and a completed "Receipts, Funds Raised and Expenses" report form must be submitted to the chapter leadership board for every hands-on service activity. The board is responsible for year-end reporting of the activities.As a best practice, though not required, you can also submit receipts and a completed "Receipts, Funds Raised and Expenses" report form for fund-raising activities. This form should be sent to the Financial Director:
