The forms listed below are for use by Chapter Leaders, Congregational Coordinators & other interested parties for requesting funds, keeping track of activities and other associated items dealing with the Walworth County Chapter of Thrivent for Lutherans.

Request for Funding Care in Communities

PDF Request for Funding, Fund Raising & Hands-on Activities
This form is to be used to request funding for Fund Raising and Hands-on activities from the Care in Communities funds.

The first two pages of this form needs to be filled out and submitted 30 days before the activity takes place. The first two need to be submitted to the Walworth County Chapter board. Refer to the document for the person(s) to submit it to. The Board meets once a month near the beginning of the month so advanced planning is essential to receive appropriate attention.

Fill the form out as accurately as possible describing the activity in the best detail possible. Contact a Chapter Leader or Thrivent Representative with any questions. See Meet Our Leaders page for contact information.

The other pages of the document give valuable information you will need prior to receiving a full packet of material after being approved at a board meeting. These are pages, 3 & 4, are can be download from the next section under Request for Funding Pages 3 & 4.
