This form needs to be filled out and submitted 30 days before the activity takes place and submitted to the Walworth County Chapter board. The Board meets once a month near the beginning of the month so advanced planning is essential to receive appropriate attention.

Fill the form out a accurately as possible describing the activity in the best detail possible. Contact you Congregation Coordinator or Chapter Leader with any questions.
