STEP 1 - To Request Funding
Since the number of Volunteer Hours helps to determine the amount we approve, PLEASE include ALL hours that will be associated with your activity (planning, preparation, set-up, work the day of the event, clean-up, etc.). The first form below is REQUIRED to request any "Care Abounds in Communities" funding from our Chapter and the second form MAY be used if pre-funding is needed for a Hands-on work service activity:

STEP 2 - To Report Completed Activity
This form MUST be submitted on-line and your "Net Proceeds" check mailed to our Financial Director along with a completed "Care Abounds in Communities Receipt Envelope" (you may use the On-line Form below in lieu of this envelope) in order for your Recipient to receive the funding approved by our Chapter:

STEP 3 - Receipts, Funds Raised and Expenses Report
This form replaces the "Receipt Envelopes" used for BOTH "Care in Congregations" and "Care Abounds in Communities" programs and MUST be submitted on-line. For fund-raisers only, your "Net Proceeds" check MUST be forwarded to our Financial Director for any "Care Abounds in Communities" activity. For ALL activities, actual receipts or copies thereof, MUST be forwarded to our Financial Director in order to comply with IRS regulations!
