How to Request Funding
Funding requests are reviewed at the chapter's monthly meetings, which are held the first Monday of each month. All funding requests MUST be received by the Duluth Area Chapter a minimum of 30 days prior to the planned event.

The form can be completed while you are on-line. Remember to save the information and then send the completed form as an attachment in an email to Diane Koranda. Also print a copy of the completed form for your files.

If you are unable to complete the form on-line you will need to send it through postal mail to Diane Koranda.

Please download this form to record attendance at your event. This information is needed to complete the "Results" form.

Use AFTER Your Event Within 45 Days
Finally, download and complete this form within 30 days after your event. The form can be completed while you are on-line. Remember to save the information and then send it, along with receipts and your check for the total amount raised to John Nys. Print a copy of this completed form for your files.
