Funding requests are reviewed at the chapter's monthly meetings, which are held the first Monday of each month. All funding requests MUST be received by the Duluth Area Chapter a minimum of 30 days prior to the planned event. |

Next, fill out the Request for Funding Questionnaire. The form can be completed while you are on-line. Remember to save the information and then send the completed form as an attachment in an email to Diane Koranda. Also print a copy of the completed form for your files. If you are unable to complete the form on-line you will need to send it through postal mail to Diane Koranda. |
