The following two forms must be forwarded to a chapter leader before your activity can qualify as a Join Hands Day Activity. Please see deadline dates listed below. (These forms are posted here for your convenience but can also be found at the above link.)

1. When you begin planning your activity, complete and submit the following form to your chapter leadership board by June 15 to request funds.
