The following forms need to be completed to conduct a Care Abounds in Communities project.1. Request for Funding
2. Activity Results Reporting
3. Other forms that may be needed, i.e. donation forms, permission to disclose, etc.
The "Funding Request" form should be sent to Community Service Team Director, Colleen Putnam.
At the completion of the project, all other forms should be sent to Financial Director, Jean Heston. Funding will not be paid until all forms and receipts have been received.
