Before your activity begins, you'll want to check out our Care Abounds in Communities Planning Checklist. While you're here, download a copy of the funding request form and information disclosure forms if applicable.
When the forms are complete, send them to our Thrivent Board Community Service Team Director. That individual will submit the application to the local Thrivent Board for review.
Use the following forms after your activity is complete. Your Thrivent Board and Thrivent need complete documentation to expedite the processing of your project.
