BEFORE THE ACTIVITY
To request funding, a 'Community Service Team Request for Funding Questionnaire Care Abounds in Communities Form' needs to be completely filled out and sent to
Judy Raffin 30 days prior to activity.
You may get the form one of two ways:
1. Contact Judy Raffin (602 Depot Street, Manawa, WI 54949, 920-596-3917, or jjraffin@wolfnet.net ) She will mail you the information and forms you will need.
2. Visit the resources website and print it off and complete. (on the link below, it is the PDF form from the 'Forms to use before your activity' side called "For Funding Request Updated for 2008"
Once the form is completely filled in, send it to Judy Raffin at 602 Depot Street, Manawa, WI 54949.
Completed forms need to be sent to Judy 30 days before the event for the board to be able to review and approve them.
If funding is approved for your activity, you will receive a letter and a packet of information in the mail. It is extremely important that you advertise your event with "Supplemental Funding Provided by the Northeast Waupaca County Chapter of Thrivent Financial for Lutherans".
During The Activity
Have fun conducting your activity--take action photos to include in the press release that gets sent to the paper or our scrapbook.
AFTER THE ACTIVITY
1. Complete the "Activity Results Reporting" document that was sent to you in your packet of information from Judy.
2. Send proof of advertisement that shows Thrivent Financial supplemented your event as stated above. Also any action photos for our scrapbook.
3. Send completed forms to: Betty Klotzbuecher, 806 Meadow Lane, Manawa, WI 54949
