If you are submitting your activity to more than one chapter, expected
member households and dollar counts must be prorated. This worksheet
will make it easier to calculate how to distribute between chapters.
Please include the completed top portion of the form with your request.

Permission to Disclose Information form (PDF)
Recipients
(those who receive the help) must complete this form giving Thrivent
Financial for Lutherans permission to use, disclose and publish the
recipient's name and other pertinent information about the recipient
for the purposes of planning, conducting, advertising and communicating
the results of a fund-raising activity for the recipient's benefit
(e.g., to publicize the fund-raising activity). If the recipient is a
qualified nonprofit organization, the form does not need to be signed
unless a person's name or likeness is published, which could be an
employee or volunteer of the nonprofit organization.
Acknowledgement of Donation Received form (PDF)
Chapter service teams should provide a written
acknowledgement/receipt for all donations valued at $25 or more made to
the chapter, including Thrivent Builds.
Special reporting is
required for donations valued at $250 or more. Information is included
on the form for tracking and reporting.

Form to fill in and submit to Twin
Peaks Chapter after activity is complete. Mail completed form, along
with funds raised and the Receipts, Funds Raised and Expense Report to
the Twin Peaks Financial Director (address below).
A common error that needs your attention is that donation checks must be made out to Thrivent, not the ultimate recipient. The checks are submitted to the Thrivent Chapter. Thrivent deposits these in its account and returns a check in the value of the funds raised plus grant amount. Audit requirements specify that grants cannot be made if the raised funds are not handled in this manner

Financial tracking
form for listing Gross Funds Raised, minus Expenses, and Net Funds
Raised. This form, along with the net funds raised at the activity and
expense receipts (or copies of receipts) is required to process the
report...even if you do not claim expenses.
