Cabarrus Stanly County Chapter of Thrivent Financial for Lutherans

_____ 1. Congregation has all 3 Congregation Coordinators (CCs) recorded

_____ 2. One Congregational Coordinator is designated as “lead”

_____ 3. Church uses all the Care in Congregations funds

_____ 4. Congregational Coordinators receive training from the chapter

_____ 5. Three projects were conducted in the Care in Congregations program `

_____ 6. A space (bulletin board, brochure holder, display) to display Chapter
Thrivent Financial for Lutherans applications, information, and other Thrivent
congregational resources is set up and maintained at the church.

_____ 7. A service team submits to the Chapter Board and completes a
Care Abounds in Communities project

_____ 8. One of the Congregational Coordinators orders the baptismal
medallions and confirmation crosses

_____ 9. A minimum of 2 announcements about Care in Congregations or Care Abounds
in Communities projects appeared in the church bulletin or newsletter

_____ 10. Thrivent benefits coffee hour is presented at church by CC (1 point) or by
Thrivent Financial Representative (2 points)

_____ 11. CC contacts a Financial Representative to present a Care in Congregations
check to church at least one time during the year

_____ 12. A CC has contacted a Financial Representative to make a presentation or seminar

_____ 13. Congregation has been involved with a local Thrivent Builds, Habitat for
Humanity program

Congregation of Excellence will be awarded for a total score of 10 points or more.
To be eligible for award, CC must turn in this form to chapter president prior to January 15th.
