You may access these forms by clicking on the links below this
letter.
Applicants must return ten (10) copies
of their application and resolution forms. All ten (10) copies must contain the
signature of the zone president, plus the address and telephone number of the
person preparing the form. Approval of the grant shall be given by the
Executive Board of the Zone in which it originates, except those submitted by
the Ohio District LCMS. (See Guidelines 1B.)
Completed Application and
Resolution forms must be returned to Leslie Lindsey, LWML Ohio District Vice
President of Gospel Outreach, and postmarked no later than October 31, 2011.
Applications postmarked after this date will not be accepted. If you need
additional forms or have questions, please call me at 419-267-5180.
Please remember our committee in your
prayers, asking that the Lord will guide us as we gather mission grants
information and choose the grants for the ballot.
